PDF only showing 5 of 9 columns - Page 1

I am using HTML2PDF for the first time. I have embedded the code into a form that creates a full page with a 9 column table. The PDF that gets created by HTML2PDF is only converting the first 5 of the 9 columns, so I am only seeing the left half of the form Is there a way to force HTML2PDF to convert all 9 columns? I can provide a copy of the html if needed.

Has anyone had a simliar problem with the entire table not getting output to the PDF?
Is there anything that I can do to force html2pdf (v 4.03) to include my entire document in the PDF?
I have attached the PDF and the HTML source.
