Hey Christophe,
I looked at the manual and it has copyrights in it stating not to duplicate it, so I will not be able to send you a scan. However, I'll try to give you as much pertinent info as possible:
1. Jumper 5 (JP5) on the blaster board indicates the I/O address. The default I/O address is 260H. With this configuration you should have the left most jumper open, and the two right jumpers connected. It should look like:

2. The CD Drive ID jumper needs to be set to 0. All other jumpers must be open:

3. The CD-ROM needs to be connected to the port on the 3DO blaster. The audio connector needs to be connected from the CD-ROM drive to the "Audio In" connector on the blaster.
4. The "Audio Out" connector on the blaster needs to be connected to the "CD Audio Connector" on your Sound Blaster card.
5. You need to connect a VGA Feature Connector Cable between your video card and the blaster. On your blaster the rightmost pins represent pin one. Usually video feature connector cables have a dark colored wire, which is where you will want to connect pin one into.
6. You need to connect a VGA Loopback Cable from the blaster's VGA Input port (bottom-most video port on the card), to the VGA output port on your video card.
7. You need to connect your monitor to the VGA output port on your blaster.
That covers the hardware setup. Here's some info about configuration:
A. Config.sys mods:
i.Add the following line before the CD-ROM drive's DEVICE statement:
DEVICE=C:\3DO\CD3DOBLT.SYS /P:260
Add the following /P and /T arguments to your DEVICE statement for the CD-ROM:
DEVICE=path\SBCD.SYS /D:MSCD001 /P:260 /T:2
Note: 260 specifies the I/O address. If you use a different I/O address you will need to change it.
B. Autoexec.bat mods:
Add the following statement:
SET 3DOBLST=C:\3DO A:260
Where C:\3DO is where you installed the 3DO driver software, and 260 is the I/O address you use
