Blank page, no form... - Page 1

For some reason the HTML2PDF is not rendering the PDF... and I have no idea why...

(The links will lead you to the code im trying to use, so you can test it, you will only need the HTML2PDF Class)

can somebody tell me why the form isn't rendering??? I'm freaking out!!!

what is the output you get ?

Si moins de monde enculait le système, alors celui ci aurait plus de mal à nous sortir de si grosses merdes !

the pdf is being generated but the form gets ignored...
